When instructed to by the LIYSF office please fill in your details in the form below and submit.
This form should ONLY be completed when you have a confirmed booking to attend LIYSF.
To book a place you must first apply to attend LIYSF by submitting an LIYSF Booking Form. After you have submitted your booking form, your application will be assessed and your referees checked. If your application is successful, you will be offered a place to attend and given a booking reference number, you must submit a booking deposit to secure your place before filling in the below form.
