Student Details
When instructed to by the LIYSF office please fill in your details in the form below and submit. This should be done when you have a confirmed booking to attend LIYSF. To book a place you must first apply to attend LIYSF by submitting an LIYSF Booking Form.
When you have completed and submitted this student details form, a copy is automatically sent to our office. Once you have completed the form please email a passport size photograph to
enquiries@liysf.org.uk
